Where to File
After your medical history, physical examination, and all required laboratory results are complete, the Civil Surgeon will prepare and sign your Form I-693. You will receive the completed form in a sealed envelope. Do not open the envelope. USCIS will not accept Form I-693 if the sealed envelope has been opened, altered, or damaged.
